We encourage all persons with comments about our performance to let us know. All comments are looked into and if we have your contact information you will be notified of the outcome. We accept anonymous complaints. Fields marked with * must be filled in for the form to be submitted.
NC State University provides an on-line Concerning Behavior Reporting Portal for our community to report observations of behavior that might indicate that a person is a danger to themselves or others. If this is an emergency, please call 911. If you are unable to make contact by phone, or wish to make an anonymous report, the use of this report is permitted. If it is at all possible to report by telephone we would encourage you to do so. In order to properly assist you, please answer the questions as completely as you can. By sharing your concerns, you are helping to maintain NC State as a healthy and safe campus environment.

CAMPUS SECURITY AUTHORITY INCIDENT REPORT
The form should be used by Campus Security Authorities (CSAs) to report crimes under the Clery Act that they have knowledge of but were not formally reported or investigated by the Police Department. This is an anonymous form that allows the Police Department to more accurately track the number of crimes that are actually occurring for Clery reporting purposes.
Completed and approved police reports are available in 3 to 5 business days. If your report is not listed, please check later.
If you have been trespassed and wish to appeal, you must file the appeal through the linked on-line form. You must completely fill out all required information and your appeal will be routed to the department that is responsible to hear your appeal.
The University Police Department provides the OCR system for our community to report miscellaneous and minor crimes or incidents. If this is an emergency, someone is injured or a crime in progress please call 911. In order to properly assist you, please answer the questions as completely as you can.
